    Have your health insurance details handy, including the name of your health plan and your member ID number.

  2. Health Condition Information
    Be ready to provide information about your health condition(s) to help determine your eligibility and the best meal options for your needs.

  3. Contact Information
    You’ll need to provide your current contact information, including your phone number and email address, so we can reach you with updates.

  4. Diet Assessment
    As part of the application, you will need to complete a Diet Assessment to help us customize your meals based on your specific needs and preferences.

  5. Provider Referral
    In some cases, your health plan may require a referral from your healthcare provider. If so, we’ll let you know and guide you through the next steps.
