Your Health Condition(s) and needs
​The benefit is generally available to individuals with chronic health conditions and or those who have been recently discharged from the hospital or need extra support. Your health plan will determine eligibility based on your condition, care coordination needs, recent discharge status, and plan coverage.

Referral (If Needed)
​For some health plans, a referral directly from your healthcare provider may be required. If this applies to you, we’ll let you know and guide you through the next steps.
